I think what Phillip is saying is that we aren't sure if Trans is biological state (akin to sexuality) or a mental illness (akin to schizophrenia). If the former, then gender dysphoria can be cured by gender affirming care and is a rights based issue. We're now doing what liberal society did with homosexuality across the last 50 years. This is what most activists want to see happen.
However, if gender dysphoria is actually caused by chemical imbalances then it is a mental condition, which can be managed and medicated using drugs. Where it gets confusing is situations where gender-dysphoria presents itself but the person presenting may not be trans. This was at the heart of what caused the closure of the GIDS at the Tavistock. Studies showing co-morbidity with the autistic spectrum is adding further complication.
We do know there is an issue here - what we don't have is answers. Yet everyone is insisting their argument is simple and correct. A complicated picture.
